What Is SMART Mercury Removal and Why Does It Matter?

Replacing a missing filling can become a bigger decision than most patients expect once mercury enters the conversation. Dental amalgam, the silver colored material used in fillings for more than a century, is roughly half elemental mercury by weight, and while federal regulators consider it safe for most adults, patients who want it removed still deserve a process that limits their exposure rather than increases it.

At Mesa Dental, we approach SAFE Mercury Amalgam Removal the same way we approach the rest of whole-health dentistry, with a protocol designed to minimize risk rather than rushing the appointment. Dr. Emily Browner holds certification through the International Academy of Oral Medicine and Toxicology, and she is one of a limited number of dentists in the Summerlin area who has completed this training.

What Does SMART Stand For?

SMART is short for Safe Mercury Amalgam Removal Technique, a protocol developed to reduce a patient’s exposure to mercury vapor during the removal process itself. Amalgam fillings release small amounts of mercury vapor when they are drilled, so the technique focuses on containing and clearing that vapor before it can be inhaled or absorbed.

The protocol was not invented as a marketing term. It reflects specific steps that a dentist takes before, during, and after removal, and it requires training beyond a standard dental degree to perform correctly.

How We Handle Removal Differently

When we remove amalgam using the SMART protocol, we build in several layers of protection that a routine filling replacement does not include. We isolate the tooth with a rubber dam so mercury particles and vapor cannot reach the back of the mouth or be swallowed, and we run a high-volume suction system close to the tooth the entire time we are working.

We also supply an alternative air source so patients are not breathing air from the immediate treatment area, and we cut the amalgam into larger chunks rather than grinding it into a fine dust whenever possible. Before the appointment even begins, we walk patients through what to expect so nothing feels rushed or unfamiliar once the process starts.

A few of the specific protections we use during a SAFE removal include the following:

  • A non-latex rubber dam to isolate the tooth being treated
  • High-volume suction positioned directly at the tooth
  • A separate air supply for the patient during removal
  • Immediate surface cleaning to clear any remaining particles

These steps do not change how the replacement filling looks or functions. They change how much mercury exposure occurs while we get there.

Who Tends to Ask About SMART Mercury Removal?

We get questions from all types of patients about removing their mercury fillings, including the following:

Patients Focused on Whole Health

Many patients who ask us about amalgam removal already think of their oral health as part of their overall health, not as a separate category. We talk with these patients about preventive dentistry and how the mouth can reflect what is happening in the rest of the body, which fits naturally into a conversation about removing a mercury-containing material.

Patients Replacing Old or Failing Fillings

Other patients come to us because an old amalgam filling is cracking, leaking, or simply reaching the end of its lifespan, and they want it replaced with a tooth colored filling rather than another amalgam. In these cases, the SAFE protocol matters just as much, since a failing filling is more likely to release mercury when disturbed than a fully intact one.

What Replaces the Amalgam?

Once the old amalgam is removed, we typically replace it with a composite or ceramic material that matches the surrounding tooth. Patients considering broader changes to their smile sometimes ask us about this while exploring cosmetic dentistry options, as replacing dark, aging fillings can noticeably brighten a smile on its own.

We walk through material options with each patient individually, since the right choice depends on the tooth’s location, the size of the filling, and what the patient wants the end result to look like.

Dr. Browner built her approach to amalgam removal around the idea that patients deserve a full explanation, not just a procedure. We take the time to answer questions about the process, walk through what a consultation covers, and make sure every patient understands why each safety step matters before we begin.
